Hope turns to tragedy for miners in New Zealand

Authorities have told families of the 29 NZ miners trapped that none of them would have survived the second blast. For five days a wave of grief had been held at bay by the sustaining power of hope in the small mining communities around Greymouth on the west coast of New Zealand's South Island.

Then, in one raw moment yesterday afternoon, all hope was dashed for relatives and friends of the 29 missing miners. A second explosion at Pike River, bigger and uglier than the first, ripped through the mine. Perhaps the men were already dead; certainly, they are now.

Drugs and Alcohol in the Workplace DVD


Drugs and Alcohol are a problem in the workplace. The object of this program is to identify some of the problems and to look at a variety of options available to handle this issue.

These are just some of the facts:

- Of all people with alcohol or other drug-related problems, 60% - 70% are in full-time employment
- Of all workplace fatalities 15% - 30% are related to alcohol and other drugs
- Of all workplace accidents 20% - 30% involve people in an intoxicated state
- People with alcohol and drug problems have 200% - 300% more absenteeism

Included in the program:

- Definition of what is a workplace alcohol and drug problem
- Safety and issues associated with alcohol and drug problems
- Identification of problems
- Issues of confidentiality
- Drug and Alcohol Testing
- Self-denial

It is commonly agreed that at least 10% of the entire workforce suffers from some form of drug dependency problem. This 10% figure is also seen by most to be on the increase.

Evacuating the biggest airliner in under 90 seconds



This is a video of the testing of the emergency exits and evacuation the Airbus A380. In order for the aircraft to get its safety certification, over 800 people needed to be safely evacuated in under 90 seconds. Enjoy!

Working at Heights shocker!

These pictures were taken by one of our own staff. Let's run off a few of the obvious things neglected here:
  • No scaffolding
  • No fall protection
  • Wet and extremely windy conditions
  • The lifting 'device' is a vice-grip tied to a rope

Given the work practice here, coupled with the windy conditions, the wind could have easily taken hold of the sheet metal and pulled the man of the top of the building. The improvised vice-grip device could have also given way, sending the sheet down to the man below.

Accident Investigation DVD 2010



There are laws and regulations that require the investigation and reporting of certain types of accidents. Insurance requirements also lead to many Accident Investigations as do potential claims at common law.

However, from an Occupational Health & Safety point of view, the main aim of Accident Investigations is not to fulfil these legal and insurance obligations, but rather to prevent recurrences of the same or similar accidents. Accident Investigations therefore should be regarded as an important part of accident prevention.

Overhead Crane Shocker


This mobile crane is holding a machine which weighs approximately 14 tonnes about 300mm off the ground. Wooden blocks, that could have been used to minimise any failure, were within easy reach.

Never work under a suspended load, even for short periods or for minor work, as the consequences of crane or rigging failure can be catastrophic. If you need to work on the load either lower it to the ground or support it on packing, such as timber dunnage.

Without oxygen being passed from the lungs to the blood stream and then pumped around the body by the heart a person will die. When a person suffers cardiac arrest their heart stops beating, they lose consciousness, they stop breathing, and blood will not circulate. Unless they receive prompt first aid attention until medical or ambulance help arrives they will suffer irreparable brain damage and die within minutes.

Kiwi jetpack invention gets ready for domestic takeoff



The chance to fly a Kiwi-invented jetpack will be available in New Zealand early next year.

Almost a year to the day after it first got worldwide attention, the Martin Jetpack is back on show at the United States' annual EAA air show in Oshkosh, Wisconsin, and Christchurch's Martin Aircraft Company is also announcing plans to allow the public to try out a "low, slow" version.

"I saw the sign"

This weeks 'I saw the sign!' is related to motor vehicles. Now for the ones of us out there that have a reasonably late model vehicle, who has seen or noticed this little tag on the seat bolster before?


Not quite sure what it is, let's go in for a closer look. . . .


As cars become safer and safer, they are fitted with more and more airbags. This little tag indicates that there is a bolster seat airbag fitted to this car. The airbag will deploy (if needed) from the tagged area. It is important we all do a quick check over our vehicles as to where airbags are fitted. This ensures we don't accidently put inappropriate seat covers or dashboard covers on etc, that could render these safety devices not only useless, but potentially dangerous.

Slips, Trips and Falls

Using a trolley as a substitute for a step ladder is a very unsafe practice. Falling, even from this seemingly low height can result in serious injury. Slips, Trips and Falls are the second most common accident category in the workplace today. This shot perfectly illustrates why. . .


Slips usually occur when there is a loss of grip between the shoe and the floor. This commonly occurs when there is a contaminant between the shoe and the floor. Trips occur when a person’s foot hits a low obstacle in the person’s path, causing a loss of balance. Often, the obstacle is not easily visible or noticed.

Sawstop - Great little safety invention!

We came across this Youtube video on 'Sawstop'. A table saw stopping system that stops a bench saw instantly when it detects an electrical signal, such as from a human finger or limb. Inventions like this (when realized into the real world) make a huge difference by adding an extra line of defence. Enjoy!

Definitive mobile phone risk assessment health study begins


The biggest study to date into the effects of mobile phone usage on long-term health has begun, aiming to track at least a quarter of a million of people in five European countries for up to 30 years.

The Cohort Study on Mobile Communications (COSMOS) differs from previous attempts to examine links between cellphone use and diseases such as cancer and neurological disorders in that it will follow users' behavior in real time.

Most other large-scale studies have centered around asking people already suffering from cancer or other diseases about their previous mobile phone use. They have also been shorter, since mobile phones have only been widely used for over a decade.

"One of the limitations of research to date is that when you ask people about their mobile phone use say, five years ago, there's a lot of error," said Jack Rowley, director of research and sustainability at European industry body, the GSM Association.

About five billion mobile phones are in use worldwide. To date, groups such as the World Health Organization, the American Cancer Society and the National Institutes of Health have found no evidence that mobile phone use harms health.

"Research to date has necessarily mainly focused on use in the short term, less than 10 years," principal investigator Professor Paul Elliott of the School of Public Health at London's Imperial College told a news conference. "The COSMOS study will be looking at long-term use, 10, 20 or 30 years. And with long-term monitoring there will be time for diseases to develop," he said.

Professor Lawrie Challis from MTHR said: "Many cancers take 10, 15 years for the symptoms to appear. So we've got to address the question: Could there be something out there that we need to look at?"

Gas bottle shocker


This photograph highlights the dangers posed when transporting gas bottles in vehicles.

In this case, the single rope holding the gas bottles failed, causing the load to become unstable and potentially hazardous. If you are transporting gas cylinders in a vehicle, ensure they are securely restrained in an upright position. One means of ensuring gas bottles are restrained, is to stow them in a built-for purpose cage or vertical racking system.

Forklift shocker!


Maybe the worker in this photo was hoping that his trousers would camouflage him during this dangerous operation!

One slip and he would join the statistics of people who have been killed or seriously injured after falling off a forklift pallet. Work at height requires access equipment suitable for the task. In this case, a forklift is not the right solution.

If you are using a forklift to lift a person, a purpose-designed man-box physically secured to the forklift is required. The operator must be at the forklift controls at all times. Both the forklift and the man-box must also meet relevant safety standards.

Safety no longer relevant! Spare body parts available from vending machines!


In the foyer of a recent safety seminar there was this rather odd vending machine. At first glance it looked like a regular food-type vending machine, but as curious onlookers neared, it could be seen that the products within were body parts! We at Safetycare loved this. A simple artistic installation that reminds us that we can't replace our body parts as simply as just buying them from a vending machine, so we had better look after our own.

Did someone say Fall Protection!?! We think not . . .

Our team came across this video on 'Camino del Rey' in Spain. This walkway was built in 1905 for workers at the hydroelectric power plants at Chorro Falls and Gaitanejo Falls. A walkway was needed to cross between the falls for inspection and maintenance of the channel.



The walkway is not maintained, and is highly deteriorated and dangerous. It is 3 feet wide, and is over 1000 feet above the river below. Almost the entire the path has no handrail and some parts of the concrete walkway have completely collapsed. All that is remaining is the steel beam originally in place to hold it up. One can latch onto a modern steel safety-wire to keep from falling, though it can't hold much weight. Several people have lost their lives on the walkway in recent years; after four people died in two accidents in 1999 and 2000, the local government closed the entrances. However, many adventurous (and crazy) tourists still find their way onto the walkway to explore it.

Elevating Work Platform (EWP) Shocker!


This shows the use of an elevating work platform (EWP) on uneven ground.

Scissor-lift EWPs must not be operated on uneven ground unless outriggers are used. They definitely should not be chocked as shown. If the surface is uneven, relocate the EWP to flat ground. Use a knuckle-boom lift or review what other height access can be employed.

Safe work practices:
- Train operators to operate equipment safely
- Ensure the supporting surface is firm, clear of obstructions and wheels or outriggers are contacting the ground before elevating or repositioning the unit
- Test equipment before operation to ensure all safety devices are fully functional
- Ensure all level indicators are in place and fully functional.

Accident / Incident Investigation video 2010



There are laws and regulations that require the investigation and reporting of certain types of accidents. Insurance requirements also lead to many Accident Investigations as do potential claims at common law.

However, from an Occupational Health & Safety point of view, the main aim of Accident Investigations is not to fulfil these legal and insurance obligations, but rather to prevent recurrences of the same or similar accidents. Accident Investigations therefore should be regarded as an important part of accident prevention.

Without oxygen being passed from the lungs to the blood stream and then pumped around the body by the heart a person will die. When a person suffers cardiac arrest their heart stops beating, they lose consciousness, they stop breathing, and blood will not circulate. Unless they receive prompt first aid attention until medical or ambulance help arrives they will suffer irreparable brain damage and die within minutes.

Manual Handling Joke:
During a manual handling training session, a strong young man at the construction site began bragging that he could out do anyone in a feat of strength. He made a special case of making fun of the site safety team. After several minutes, the Safety Manager had had enough.''Why don't you put your money where your mouth is'', he said. "I bet you a weeks wages that I can move something in a wheelbarrow over to that site hut that you won't be able to wheel back". "You're on, old man", the braggart replied. "Let's see what you got". The Safety Manager grabbed a wheelbarrow by the handles. Then, nodding to the young man, he said, "All right, Get in".

Fire Drill
