Scaffold shocker


 Obvious problems

• Scaffolds too low for the work height
• Standing on the guard rail
• Guard rails too low should be at least 900mm from the platform
• No internal ladder access to work platforms
• Scaffolds not erected to manufacturer’s instructions

Hidden problems

• Poor job planning
• Poor worker training
• Lack of effective supervision

Texas: Fertilizer plant explosion near Waco



Up to 70 people have been reported dead in a fertiliser plant explosion near Waco, Texas. The Emergency Medical Services director in the town of West, Dr George Smith, told the local KWTX station as many as 60 or 70 people had died and hundreds were injured in the blast on Wednesday night.

Our thoughts are with the community and families of those affected.
