Scaffolding platform shocking photo

The use of barrels to hold up this scaffolding platform is another example of poor planning and dangerous innovation. The barrels are not suitable scaffold foundations and simply cannot support the weight of the scaffold and loads on it, such as bricks and people.
Other bodgey extras include:
• welded barrels
• loose bricks around the site
• unsecure ladder
• single-plank working platform
• no fall protection/guardrail
How hard can it be? Take time and organise the right equipment for the job prior to starting work.

Grinder Shocker

Thought you’d seen the last of these killing machines? This had numerous faults including:
- No guard
- The second handle had been removed
- The manufacturers plate could not be read, so we do not know the correct RPM of the grinder
- The cable was damaged and taped over
- The unit was not tagged and tested
- The body of the unit was damaged and the front and rear sections were twisting all over the place.

This graphic warns of the danger of wearing loose fitting clothing around a bench drill press. It is important to never become complacent or forgetful of what you are wearing when using a bench drill press, lathe or any other rotating/moving piece of machinery. The potential entanglement accident can be quick and deadly. This particular hazard summary is from the Ministry of Labour, Ontario, Canada:

"A lathe operator was fatally injured while polishing a slotted steel shaft with emery paper. Before putting it in the lathe, he had machined three keyway slots along the 3" diameter shaft. After securing it between the chuck and the tailstock of the lathe he began polishing with a long strip of emery paper looped around the rapidly rotating shaft. The slots caught the strip of paper, which entangled his gloved hand and pulled his arm around the shaft, severing it from his body. He died later from extensive head, neck and chest injuries."

At first glance it looked like a regular food-type vending machine, but as curious onlookers neared, it could be seen that the products within were body parts! We at Safetycare loved this. A simple artistic installation that reminds us that we can't replace our body parts as simply as just buying them from a vending machine, so we had better look after our own.

Electrical Safety Shocker!

This 15 amp heavy-duty extension lead was brand new and had been borrowed from a principal contractor by a sub-contractor. The lead had been inspected, tested and tagged prior to the sub-contractor borrowing it. During a workplace inspection a couple of days later, the lead was seen to be incorrectly installed into one of the construction switchboards (it wasn’t secured to prevent it being pulled out).

When checked this dangerous modification was found to the plug top. As the board was only fitted with 10 amp socket outlets and the 15 amp plug would not fit – someone simply broke off the extra large earth pin rather than sourcing the proper lead.
